Division Functions

 

  1. To develop and maintain personnel information system; 
  1. To prepare and authenticate correspondence from units as well as those for use in other government agencies and the private sector; 
  1. To publish/distribute/disseminate: 
  • Rules and regulations pertaining to PNP;
  • Rules, Routine Orders, Bulletins, Circulars, and all miscellaneous publication;  and,
  • Special Order, General Orders, Letter Order, and other Directives issued by the National Headquarters. 
  1. To prepare, administer and supervise the issuance of PNP Identification Cards and Badges; 
  1. To administer, direct and supervise the operation of the Postal System in the National Headquarters; 
  1. To maintain and preserve all office

PNP PERSONNEL

Duly accomplished PNP ID Application Form [Download Here] with the following:

A. NEW APPLICANT
     1. Unit Endorsement
     2. Appointment Order
     3. Authenticated Payslip (Latest)

B. RENEWAL / UPDATE
     1. Unit Endorsement
     2. Surrender Old / Expired ID
     3. Promotion Order, If Promoted
     4. Demotion Order, If Demoted
     5. Change of Status / Amendment Order, If Amendment / Changed

C. REPLACEMENT (loss / dilapidated)
     1. Unit Endorsement
     2. Affidavit of Loss, If Lost
     3. Surrender Dilapidated PNP ID, If Dilapidated
     4. Payment of 60.00 pesos for the Replacement ID Card

PNP DEPENDENT
Duly accomplished PNP ID Dependent Application Form [Download Here] with the following:

     1. Application form Endorsed by Admin / Pers Officer
     2. Certification of Legal Beneficiaries (CLB) from Current Record Section 
     3. At least one (1) valid government-recognized ID
     4. Affidavit of Loss, If Lost
     5. Payment of 60.00 pesos for the Dependent ID Card

>> Vacation/ Sick Leave (Abroad) and Study Leave
  For Vacation/Sick Leave Abroad
     1. Endorsement from concerned unit
     2. Basic Letter Request, including the number of days, reason for Leave and address while on leave
     3. Duly accomplished CSC Form No. 6 item 7A sign by C, RMD and 7B sign by Rds for PROs and Directors for NSUs 
     4. DLOD Clearance
     5. Unit Clearance
- Certificate of Non-Pending Case
- Certificate of Non-Money/Property Accountability
6. Affidavit of undertaking that the personnel is not a witness in a drug related case, otherwise the personnel has
to secure a permission from the court.

7. Medical Certificate (if applying for sick leave)

     For Study Leave
     1. Letter Endorsement from Cjief Office/Unit
     2. Personal Letter request
     3. Duly accomplished CSC Form No. 6 item 7A sign by C, RMD and 7B sign by Rds for PROs and Directors for NSUs 
     4. Latest Service Records from DPRM
     5. IPER (2 quarters)
6. Attested Permanent Appointment
7. Transcript of Records/Diploma
8. Non-Pending Case
9. DLOD Clearance
10. Affidavit of undertaking
11. Certificate of no outstanding service obligation from previous Foreign/Local Scholarship/Training grants attended
12. Receipt from Review Center
13. Medical Certificate (Physically and Mentally fit)
